Purpose of Exercise: Use these questions to evaluate your client’s development progress.

How to Administer and Use this Exercise to Facilitate Behavior Change: I often tell my coaching clients… behavioral change is often a process of “trial and error”. When we learn about what has worked or what hasn’t, we are able to adjust our developments plans accordingly. It is important to track progress to ensure that individuals are on the right path of their development plan. This coaching evaluation questionnaire will help you to assess (overall) the extent to which an individual has changed. This assessment is useful not only for coaches, but for managers and or other stakeholders in an organization that are able to assess a client’s progress.
